Send A Traditional Case

Completely fill out and sign the prescription. Please check all required boxes to ensure your Rx specifications are met. Then, carefully package your case, including your prescription, and tape box securely shut. We ask that you follow these steps when packing your case:

1. Fill out a RX Form

2. Place padding on bottom of box

3. Separate models, and secure impressions and bites

4. Arrange contents in a plastic bag for proper infection control

5. Place padding on top of contents

6. Add prescription

7. Tape box securely shut

8. Send to Perfect Smile

When bundling cases, separate the cases in plastic bags and staple prescription to the outside of each bag for accurate identification. Please call Perfect Smile Dental Ceramics at (858) 467-9449 and we will be happy to help you. All rush cases must be pre-approved and pre-scheduled. If you are sending a prescheduled rush case, write RUSH on the outside of the box/package, as well as on the prescription.
